Required Forms
Each referee is required to fill out the following forms before they can referee U08-U13 recreational games for EYSA through www.EYSAReferees.org.
1) Risk Management Form (on-line) - All referees are required to submit this form to Washington State Youth Soccer Association (WSYSA)
2) EYSA Parental/School Permission Form - All referees under 18 years old are required to submit this form to your Referee Coordinator. Choose the form that best describes your job situation. Each form will be highlighted with the areas you need to fill out. You can contact a school counselor, athletic director or vice principle for the school's signature.

Please note: EKCSRA requires a similar but different Parent/School Permission form if you are going to referee games through them. You may not use this form for EKCSRA and you can not use EKCSRA's form for EYSA. (EKCSRA provides referees to Premier, Select and U14+ Rec games.)
3) Proof of Age - If you are under 18 years old, your Referee Coordinator needs a proof of your age - usually a copy of a birth certificate or passport.
4) Independent Contractor Form
All referees for Bellevue, Issaquah, Mercer Island and Newport need to sign an independent contractor form with each club they referee games for acknowledging they are independent contractors and not employees. The form must be turned into the Referee Coordinator for that club.
